A quiz on your own site that scores what people tell you. It is versioned, so editing it never rewrites what somebody already said — which is the difference between data and a form.

One submission, and more than one answer about it.
A quiz does not have to produce one number. Answers can score across several dimensions at once, so a person can be a strong fit on budget and a weak one on timing, and you can see both rather than an average that hides them.
There is a single total as well, and a result key — the named outcome a set of answers lands on, which is what you actually act from.

The questions
Yours, and weighted the way you would weight them.
You write the questions and decide what each answer is worth. Nothing here has an opinion about what a good enquiry looks like in your field, because it could not possibly know.
Every response is kept with the answers, the score, the category scores and the factors that produced them — so a number you disagree with can be opened rather than argued with.
Where the answers go
Two places, and one of them appears by itself.
A response attaches to a lead, and to a customer account if the person has one.
The pipeline switches itself on
Publish your first quiz and the Leads module appears without being asked. Answers with nowhere to land are half a feature, so it is not something you have to notice and go and enable.
It joins the person, if they are known
If the person answering already has an account, the response attaches to it. Their quiz answers sit next to their orders and their bookings rather than in a separate list.
Locale is recorded
Which language they answered in, kept on the response — so a quiz running in English and Arabic does not blend two audiences into one undifferentiated pile.
How you get it
It is already on when you get the dashboard
This one is not on a checkout page, and you do not have to ask for it. Module flags are set while your dashboard is being built — if the site needs it, it is switched on before you ever log in. Nothing extra on your plan, and nothing to request.
If your site qualifies enquiries, quizzes is switched on during setup — and the Leads pipeline appears by itself the moment the first quiz is published.
Before you start
What this does not do, said now rather than later.
It is not a form builder
A quiz scores. If you want a plain contact form with a message box, that is the forms module and it is already on.
No conditional branching
Everyone sees the same questions. There is no showing question five only to people who answered question two a certain way.
No email sequences off the back of a result
A response is recorded and scored. Following it up is something a person does, and nothing here will nurture anybody for you.
